Drug safety reporting
While we are continuously seeking for total safety of our products it is
never fully attainable. Unfortunately, there is no way of knowing
if a patient is going to experience a side effect or an adverse event to a medication.
Should you experience any side effect or adverse event, kindly take a moment to fill out
the online reporting page, this way we can monitor the outcomes and
offer the best advice to those using Crux or any of our partner products.
We encourage you to report at least the minimum information as soon as possible.
The initials of the person who experienced the possible side effect and/or other identifiers, such as gender, date of birth.
The contact information of the reporter, namely the person who is reporting the issue.
A description of the possible side effect itself, such as the signs and symptoms experienced, date the possible side effect started and the result of the possible side effect.
The name of the product involved.
Always keep track of the event and inform your physician and pharmacist as soon as possible.

The reporter hereby provides consent to the processing of personal information provided for the purpose of reporting a suspected adverse reaction. The reporter acknowledges that this information may be used for:
a) To access medical and clinical records for the purpose of gathering additional information for clinical meaningful data
b) Generation of statistics
